The first step in initiating services and accommodations is a meeting
between the student and the Disability Services Specialist. Then:

the student submits current written documentation from an appropriate
professional licensed to diagnose the condition. Documentation
should verify the presence and impact of the disability in an educational
setting. Students are responsible for any costs incurred related to
the professional verification of disability.

the Disability Services Specialist reviews the documentation and any
other supporting materials to certify eligibility for services. Students with insufficient documentation may be referred to
physicians, psychologists, or other qualified diagnosticians for
complete assessment before accommodations are granted. Students
are responsible for any costs incurred related to the professional verification of disability.

the Disability Services Specialist meets with the student to
determine appropriate academic adjustments and/or reasonable
accommodations.

a 'Notification of Need for Accommodation' letter
is prepared and provided to the student. The student is
responsible for providing the accommodation request to his/her
instructors.
Students seeking accommodations or services on the basis of a temporary disability
must provide documentation verifying the nature of the condition, stating
the expected duration of the condition, and describing the accommodation
deemed necessary. Such verification must be provided by a
professional health care provider who is qualified in the diagnosis of
such conditions.
